Managing a chronic or ongoing health condition can sometimes feel overwhelming, especially when multiple appointments, medications, and lifestyle changes become part of everyday life. A GP Management Plan is designed to help patients receive more structured, coordinated care with ongoing support from their healthcare team.
At Redbank Medical Centre, GP Management Plans are tailored to each patient’s individual health needs, helping create a clearer pathway for long-term care and better health outcomes.
What Is a GP Management Plan?
A GP Management Plan (GPMP) is a personalised care plan prepared by your GP for patients living with chronic or long-term medical conditions.
The plan helps organise your healthcare by outlining:
- Your current health concerns
- Treatment goals
- Medications and ongoing management
It may also include lifestyle recommendations, referrals to allied health professionals, and regular review appointments to help support your long-term health.
Who May Be Eligible?
GP Management Plans are generally available for patients living with conditions that have been present, or are expected to remain, for six months or longer.
This may include conditions such as:
- Diabetes
- Asthma
- Heart disease
- Arthritis
- Chronic pain
- High blood pressure
- Mental health conditions
- Osteoporosis
- COPD and other respiratory conditions
Your GP can assess whether a care plan may be appropriate for your individual circumstances.
How Can a GP Management Plan Help?
A GP Management Plan can help patients stay more consistent with their healthcare, better manage ongoing conditions, and receive more coordinated support over time. Regular monitoring and follow-up appointments also allow your GP to adjust your care as your health needs change.
Access to Allied Health Support
For eligible patients, a GP Management Plan may also provide access to selected allied health services through Medicare.
Depending on your health needs, your GP may recommend support from professionals such as:
- Physiotherapists
- Dietitians
- Podiatrists
- Exercise physiologists
- Psychologists
If you are living with an ongoing medical condition, speaking with your GP about a care plan may help provide clearer direction and ongoing support for your health.
